Prevention of Nausea and Vomiting
Use only third-line after all other anti-emetics have been tried. Levomepromazine is a broad spectrum anti-emetic that should only be used instead of, NOT in combination with metoclopramide, cyclizine or hyoscine. OK in combination with ondansetron. Beware of phenothiazine-like side-effects.

Child 1 year - 11 years | 0.1mg/kg od up to q12h |
Child 12 years - 18 years | 6.25mg od starting dose and titrate up to max of 12.5mg bd |
Larger doses may be sedative |
IV/SC Infusion | |
Child 1 years- 18 years | 0.1mg/kg/day as a continuous infusion |
Make up to 50ml with NaCl 0.9%, start infusion at 2ml/hr | |
Titrate to effect against nausea and vomiting and degree of drowsiness by increasing or decreasing by 0.5-1ml /hr every six hours | |
Decrease as above before stopping |
Palliative Care
Oral | |
Child 2 years - 11 years | 0.25mg/kg - 1mg/kg once to twice daily (max 25mg/24 hours) |
Child 12 years - 18 years | 6.25mg to 25mg once to twice daily (maximum of 50mg/24 hours) |
IV/SC Infusion | |
Child 1 years- 18 years | 0.1mg/kg to 0.25mg/kg/day as a continuous infusion |
Maximum of 25mg/24 hours. Larger doses may be sedative |
